As a Primary Care Physician, you will manage a dedicated panel of adult and Medicare patients, focusing on prevention, chronic disease management, and acute care. This role exists to close care gaps and enhance outcomes by leveraging data and evidence-based guidelines. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to coordinate care and ensure continuity for our patients.
In this position, you will engage patients and families in shared decision-making, delivering routine wellness visits, vaccinations, and age-appropriate screenings. You will also participate in regular team huddles and quality improvement initiatives to refine our clinical approach.
