As a Primary Care Physician, you will manage a dedicated patient panel, focusing on prevention and chronic disease management. This role exists to bridge the gap in care for vulnerable populations, ensuring that every patient receives evidence-based treatment and holistic support. You will collaborate closely with an interdisciplinary care team—including nurses, care managers, and specialists—to coordinate care and improve health outcomes. Your day-to-day involves engaging patients and families through education and shared decision-making, while utilizing EHR systems to track progress and manage your panel effectively. You will also participate in quality improvement and population health initiatives, contributing to a collaborative, patient-centered team culture.
