As a Primary Care Physician, you will manage a dedicated panel of adult and geriatric Medicare patients, focusing on prevention, chronic disease management, and early intervention. This role exists to bridge the gap in care for vulnerable populations by leveraging data and interdisciplinary collaboration to optimize health outcomes. You will work within a team-based model alongside nurse practitioners, behavioral health specialists, care managers, and social workers to address not only clinical needs but also the social determinants of health.
Your day will involve conducting routine wellness visits, managing urgent care needs, and developing individualized care plans tailored to each patient's unique circumstances. Beyond clinical duties, you will engage in quality improvement initiatives and population health management efforts, ensuring that care is driven by patient outcomes rather than visit volume.
