As a Primary Care Physician, you will join a team designed to deliver high-quality, value-based care. Unlike traditional models, this role allows you to manage a smaller patient panel, enabling longer, more meaningful visits. You will focus deeply on chronic disease management and preventative care, working alongside an integrated team of nurses, social workers, care managers, and medical assistants. Together, you will coordinate care with specialists and community resources to ensure continuity and quality for your patients.
Your day-to-day involves conducting detailed assessments, developing individualized care plans, and utilizing electronic health records to proactively manage high-risk patients. You will actively participate in quality improvement initiatives and clinical case reviews to drive better outcomes and practice efficiency. Beyond clinical duties, you will contribute to a culture of inclusivity and support the professional development of your clinical colleagues.
