As a Primary Care Physician, you will manage a dedicated patient panel focused on prevention, chronic disease management, and acute care for older adults. This role exists to close care gaps and drive better outcomes through a collaborative, interdisciplinary approach. You will partner with nurses, medical assistants, behavioral health specialists, care managers, and community health workers to deliver seamless care.
Your day-to-day involves conducting detailed history and physical examinations, interpreting diagnostic tests, and developing individualized treatment plans for conditions such as diabetes, hypertension, COPD, and heart failure. You will lead routine wellness and annual Medicare visits, ensuring patients receive age-appropriate preventive screenings and vaccinations. By engaging patients, families, and caregivers in shared decision-making, you will support adherence to treatment plans and lifestyle modifications. Additionally, you will participate in regular team huddles, case reviews, and quality improvement initiatives to continuously enhance the patient experience.
