About the Role

As a Primary Care Physician (PCP) at ChenMed, you will do more than treat sickness; you will act as a health coach. You will build trusting relationships with a panel of up to 450 patients, helping them set and achieve short and long-term health goals. Your role is to simplify care so that behavior change is actionable for both patients and your care teams.

You will lead a dedicated care team, including care promoters and coordinators, to manage acute and chronic illnesses in older adults. Whether patients are in the office, hospital, or homebound, you will ensure comprehensive care coordination and accountability for outcomes. As a leader within the organization, you will champion a positive culture and work closely with managers and medical directors to drive quality and compliance.
