As a Primary Care Physician, you will manage a dedicated patient panel, focusing on prevention, chronic disease management, and acute care visits for an adult and Medicare population. This role exists to close care gaps and improve outcomes through evidence-based guidelines and population health management.
In this position, you will collaborate closely with an interdisciplinary care team—including nurses, medical assistants, behavioral health specialists, care managers, and community health workers—to coordinate care and ensure continuity. You will engage patients, families, and caregivers in shared decision-making, delivering routine wellness visits, vaccinations, and age-appropriate preventive screenings. By utilizing the electronic health record and participating in regular team huddles and quality improvement initiatives, you will help drive a positive clinical culture and ensure compliance with regulatory and payer requirements.
