The Primary Care Physician (PCP) at ChenMed is more than a consultant for sickness; you will act as a health coach dedicated to helping patients set and achieve short and long-term health goals. You will manage a panel of up to 450 patients, demonstrating accountability for outcomes, strong clinical care, and cost-effectiveness.
In this role, you will simplify and prioritize care to make behavior change actionable for your patients and your care teams. You will lead a dedicated care team—including care promoters, facilitators, and coordinators—to ensure comprehensive management of acute and chronic illnesses in older adults. Whether patients are in the office, hospital, skilled nursing facility, or homebound, you will engage with the appropriate teams to ensure seamless care coordination.
As a leader within your center, you will champion a positive culture and work closely with managers and medical directors to ensure compliance with guidelines and participate in risk and quality management programs. You will be expected to function both independently and collaboratively, influencing your patients, teams, and yourself to achieve the best possible health outcomes.
