About the Role
As a Physician in Post-Acute and Value-Based Care, you will lead the rounding strategy across 8–20 skilled nursing and long-term care facilities. This role is designed for true schedule flexibility, eliminating the need for 12-hour shifts or a traditional clinic grind. You will create your own rounding schedule for skilled, custodial, and ACO REACH patients, performing and documenting timely visits per regulatory and clinical guidelines.
You will manage skilled patients with a minimum of two visits per week and custodial patients with monthly visits. The role involves participating in daily rounds for ACO REACH patients alongside a medical director and coordinating closely with NPs, care teams, and the call center for seamless communication and follow-up. You will also handle appropriate coding, documentation, discharge summaries, and participate in Quality Improvement (QI), Peer Review, and utilization management.
