The Medical Pod Leader serves as the clinical leader responsible for the overall performance of the clinical pod team. This includes ensuring accurate diagnosis and documentation, maintaining quality standards, enhancing patient experience, ensuring affordability and safety, and fostering strong clinical team dynamics. You will function as the collaborating physician for Advanced Practice Providers (APPs) within the pod and maintain a dyad partnership with the Market Manager and market office staff.
In this role, you will build a collaborative team-based culture, working closely with Community Health Workers, Medical Social Workers, Nurse Navigators, and Care Managers. You will lead high-risk huddles, conduct chart audits, review quality metrics (HEDIS), and partner with Regional Medical Directors to identify opportunities for care improvement. The position involves active patient care for assigned panels, particularly for high-risk or complex patients, and includes leading clinical education on diagnosis, documentation, and chronic condition management.
