About the Role

As a Medical Pod Leader, you will serve as the clinical leader responsible for the performance of your clinical pod team. This role is designed to ensure accurate diagnosis and documentation, high-quality care, patient safety, and superior productivity. You will function as the collaborating physician for advanced practice providers (APPs) within your pod, fostering a dyad partnership with the Market Manager and market office staff.

In this position, you will build a collaborative team-based culture, lead high-risk huddles, and conduct chart audits to drive quality metrics. You will act as a resource for clinical team members, participating in difficult conversations with patients and families as needed. The role involves active patient care for assigned panels, including high-risk and complex cases, as well as leading clinical education on diagnosis and chronic condition management. You will work closely with a diverse team including Community Health Workers, Nurse Navigators, and Care Managers to ensure comprehensive care plans.
