Signify Health, part of CVS Health®, is seeking a Part Time Clinician to provide In-Home Health Evaluations. This role is designed for those with a passion for engaging with people and helping them on their journey to better health. You will play a critical role in building trusted relationships to make people healthier, bringing flexible, comprehensive, and personalized health evaluations right to people's front doors.
The visit lasts about 45 minutes, allowing time to connect one-on-one and answer health questions. You will conduct a medication and medical history review and a physical evaluation. If ordered by the person's health plan, you may perform additional diagnostic tests such as diabetic eye exams or spirometry. You'll use an iPad and an intuitive clinical workflow for each evaluation. Importantly, you are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.
This role allows you to bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes. You will be part of efforts to visit millions of people nationwide, providing connections to the right care for unique needs. The position involves working with Signify Health as a clinician colleague, fulfilling the requirements of the specified role, and being flexible to travel locally and within licensed states as needed. Virtual visits may also be conducted as required.
