About the Role
We are seeking a compassionate Physician Assistant to join our primary care team. In this role, you will provide comprehensive care to Medicare-eligible seniors, managing chronic diseases and delivering evidence-based treatment plans. You will work closely with physicians, nurses, and other care team members to coordinate care and drive better health outcomes.
Your day will involve conducting geriatric assessments, interpreting diagnostic results, and educating patients and their families on self-management and medications. Beyond clinical duties, you will contribute to population health efforts and participate in initiatives that refine our care delivery. This position offers the opportunity to make a meaningful impact in a supportive, interdisciplinary environment dedicated to the well-being of vulnerable seniors.
