About the Role
As a Nurse Practitioner with HarmonyCares, you will serve as the primary clinical partner for patients who require comprehensive care within their own residences. This role exists to bridge the gap between acute hospital care and long-term home management, ensuring continuity for those with chronic conditions such as CHF, COPD, diabetes, and CKD.
In this position, you will conduct in-home assessments, manage acute and chronic conditions, and develop individualized care plans in collaboration with an interdisciplinary team. Your day will involve a blend of autonomous field work and virtual engagement, utilizing telehealth to supplement in-person visits. You will coordinate care transitions following hospital discharges, provide education to patients and caregivers, and facilitate referrals to supportive services. Requirements
