About the Role
Signify Health is seeking a compassionate Physician Assistant to deliver in-home clinical assessments and support value-based care for diverse and vulnerable populations. In this field-based role, you will perform comprehensive health evaluations, review histories, conduct physical exams, and document findings in the EHR. You will identify care gaps, chronic disease risks, and social determinants of health, collaborating with an interdisciplinary team to create patient-centered care plans.

Requirements
- verifiedCurrent, unrestricted Physician Assistant (PA) license in the state of practice
- verifiedGraduation from an accredited Physician Assistant program
- verifiedNational Commission on Certification of Physician Assistants (NCCPA) certification
- verifiedCurrent BLS/ACLS certification preferred
- verifiedValid driver's license and reliable transportation for home visits
