About the Role
As a Home Health Physical Therapist, you will partner with physicians and clinical team members to develop and implement conditioning and rehabilitation programs consistent with each patient's Plan of Treatment. Your day will involve testing and screening patient physical strengths, adjusting treatments to achieve maximum results, and providing clear instructions to Physical Therapy Assistants and Home Health Aides. You will serve as a key communicator, reporting patient progress to physicians, families, and care management staff while ensuring accurate and timely documentation of all care observations and interventions.
