As a Home Health Physical Therapist at CenterWell, you will become a vital part of our caring community, reporting to the Clinical Manager. Your mission is to help patients regain strength, mobility, and independence from the comfort of their own homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.
In this role, you will work onsite in patients' homes to plan and administer prescribed physical therapy treatments. You will assess and document patients' physical strengths to support physician evaluations and guide treatment goals. You will develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and the goals of the patient team. Additionally, you will provide clear written care instructions to Physical Therapist Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation. You will serve as a key communicator, reporting patient progress to physicians, the Clinical Manager, the care team, patients, and families, while submitting all required evaluations, plans, progress notes, and discharge summaries.
This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. As this position involves travel to patient homes, it is also part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with their residing state's minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
