About the Role
As a Home Health Physical Therapist at CenterWell Home Health, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence from the comfort of their own homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.
In this full-time, weekend (Baylor) position, you will plan and administer prescribed physical therapy treatments onsite in patients' homes. Your day will involve assessing and documenting patients' physical strengths to guide treatment goals, developing rehabilitation programs consistent with physician plans, and providing clear care instructions to PTAs and Home Health Aides. You will serve as a key communicator, reporting patient progress to physicians, the clinical manager, the care team, and families while ensuring all documentation is thorough and timely.
Hiring Process & Requirements
Candidates for this patient-facing role must hold a degree from an accredited Physical Therapy Program (approved by the APTA), a current and unrestricted Physical Therapy license, and a current CPR certification. Experience collaborating with a team of healthcare professionals is required, with one year of experience as a Physical Therapist preferred.
Please note that this role is part of Humana's Tuberculosis (TB) screening program and driver safety program. Selected candidates will be required to undergo TB screening and must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
