As a Home Health Physical Therapist at CenterWell Home Health, you will become a vital part of our caring community. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.
In this role, you will plan and administer prescribed physical therapy treatments onsite in patients' homes to attain the highest level of physical function. You will assess and document patients' physical strengths to support physician evaluations and guide treatment goals, developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment and the patient team's goals. Additionally, you will provide clear written care instructions to Physical Therapist Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation. You will also document patient observations, interventions, and evaluations promptly and thoroughly, reporting progress to physicians, the Clinical Manager, the care team, patients, and families.
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. As this position requires travel to patient homes, it is part of Humana's driver safety program. Candidates must have a valid state driver's license and maintain personal vehicle liability insurance in accordance with their residing state's minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
To be considered, candidates must hold a degree from an accredited Physical Therapy Program (approved by the APTA), possess a current and unrestricted Physical Therapy license, and hold current CPR certification. Experience collaborating with a team of healthcare professionals is required, with one year of experience as a Physical Therapist preferred.
