As a Home Health Physical Therapist at CenterWell, you will join a caring community dedicated to helping patients regain strength, mobility, and independence from the comfort of their homes. Reporting to the Clinical Manager, your mission is to deliver personalized care that focuses on functional improvement, empowering individuals to overcome limitations and enjoy a better quality of life.
In this role, you will work onsite in patients' homes to plan and administer prescribed physical therapy treatments. You will assess and document patients' physical strengths to guide treatment goals, develop rehabilitation programs consistent with physician plans, and provide clear care instructions to Physical Therapy Assistants and Home Health Aides. Your work involves collaborating closely with a team of healthcare professionals, including physicians and family members, to report progress and ensure comprehensive care delivery.
This role is patient-facing and requires participation in Humana's Tuberculosis (TB) screening program. Additionally, as part of Humana's driver safety program, you must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with your state's minimum required limits.
Candidates must hold a degree from an accredited Physical Therapy Program (approved by the APTA), a current and unrestricted Physical Therapy license, and current CPR certification. While one year of experience as a Physical Therapist is preferred, the primary focus is on your ability to collaborate effectively within a healthcare team.
