As a Home Health Physical Therapist, you will work onsite in patients' homes to attain the highest level of physical function. Your day will involve assessing and documenting patients' physical strengths to support physician evaluations, developing rehabilitation programs consistent with the physician's Plan of Treatment, and overseeing the performance of Physical Therapist Assistants (PTAs) and Home Health Aides. You will serve as a vital link in the care team, reporting patient progress to physicians, managers, families, and the broader care team while ensuring all documentation is thorough and timely.
