About the Role
As a Home Health Physical Therapist, you will work onsite in patients' homes to attain the highest level of physical function. Your day will involve assessing and documenting patients' physical strengths to support physician evaluations, developing rehabilitation programs consistent with the physician's Plan of Treatment, and overseeing the performance of Physical Therapist Assistants (PTAs) and Home Health Aides. You will serve as a vital link in the care team, reporting patient progress to physicians, managers, families, and the broader care team while ensuring all documentation is thorough and timely.

Hiring Process & Requirements
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. Additionally, as this role is part of Humana's driver safety program, you must have a valid state driver's license and maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits (or $25,000/$25,000/$10,000, whichever is higher).
