About the Role

As a Home Health Physical Therapist, you will work onsite in patients' homes within the Phoenix, Chandler, Tempe, and Ahwatukee coverage areas. Your day will involve planning and administering prescribed physical therapy treatments to attain the highest level of physical function. You will assess and document patients' physical strengths to support physician evaluations and guide treatment goals, developing rehabilitation programs consistent with the physician's Plan of Treatment and the patient team's objectives.

You will also provide clear written care instructions to Physical Therapist Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation. A key part of your role involves communicating patient progress to physicians, the Clinical Manager, the care team, and the patient's family, while submitting all required evaluations, plans, progress notes, and discharge summaries promptly and thoroughly.

Hiring Process & Requirements

This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. Additionally, as this role is part of Humana's driver safety program, you must have a valid state driver's license and maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
