About the Role

As a Physical Therapist in Home Health, you will be the primary driver of patient care within the home setting. Your role involves completing comprehensive assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on these findings, you will collaborate to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and continuously evaluate the patient's progress. You will work in coordination with other members of the healthcare team, instructing patients and families regarding the plan of care, functional limitations, and progress toward goals.
