As a Home Health Physical Therapist, you will become a vital part of our caring community. Reporting to the Clinical Manager, your primary focus is to help patients regain strength, mobility, and independence from the comfort of their own homes. You will deliver personalized care that emphasizes functional improvement, empowering individuals to overcome limitations and enjoy a better quality of life.
In this role, you will partner with patients and their families to develop and implement rehabilitation programs consistent with physician plans of treatment. You will assess patients' physical strengths, document observations and interventions thoroughly, and provide clear care instructions to Physical Therapy Assistants and Home Health Aides. Your work ensures that every patient receives the support they need to progress toward their recovery goals.
This role is patient-facing and requires a current, unrestricted Physical Therapy license and current CPR certification. Candidates must have experience collaborating with a team of healthcare professionals. A valid state driver's license and personal vehicle liability insurance are required, as this position involves driving to patient homes. All selected candidates will be subject to Humana's Tuberculosis (TB) screening program and driver safety program.
