About the Role

This Mobile Outpatient Therapy position is designed for a Physical Therapist passionate about making a meaningful difference in the future of rehabilitation and wellness. You will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population, treating patients in their private residences or within independent living facilities.

The "Powerback Rehabilitation To You" model allows you to evaluate and treat patients in the most functional environment possible: their home. Your day will involve assessing patient needs, developing individualized therapy programs to restore and enhance performance, and instructing patients and caregivers in skills essential for promoting independence. You will work collaboratively with other disciplines to address comprehensive patient needs, ensuring a safe and compassionate atmosphere that prioritizes patient service.
