This local contract opportunity places a Physical Therapy Assistant in Farmington, Maine, within an outpatient rehabilitation setting. The role focuses on serving the older adult population through in-home care, whether in a patient's private residence or an independent living facility.\n Rehabilitation To You allows the Physical Therapy Assistant to evaluate and treat patients in the most functional environment possible: their home. Under the supervision of a Licensed Physical Therapist, you will facilitate increased independence and functioning by implementing individualized treatment plans. This position offers a flexible schedule and work environment, allowing you to make a direct impact on patients' daily lives while working as part of a collaborative rehabilitation team.
Candidates must possess a valid license in the state of practice or proof of license eligibility, along with a valid driver's license or reliable independent transportation to reach patient residences. Successful applicants will participate in required orientation and training programs to ensure compliance with all applicable legal requirements, standards, and policies, including HIPAA and the Federal False Claims Act.
