This position offers a unique opportunity to provide outpatient rehabilitation services directly within the comfort of a patient's home or in an independent living facility. By treating older adults in their most functional environment, the Physical Therapist can deliver care that is deeply integrated into the patient's daily life, fostering greater independence and productivity.

The Physical Therapist will evaluate, diagnose, and design individualized treatment plans to restore, reinforce, and enhance performance. As a flexible role, the Physical Therapist will navigate various settings, collaborating with a multidisciplinary team to address the comprehensive needs of each patient. Requirements
