This position serves as a pivotal coordinator for physical therapy services across inpatient, outpatient, and home health settings. You will be responsible for the daily delivery of care, implementing specific treatment programs tailored to individual patients and groups. Your day involves evaluating patients through a variety of data sources, developing and reviewing individualized plans of care, and instructing patients, families, and caregivers on home exercise programs.
Beyond direct patient care, you will function as a leader within the department, overseeing physical therapy assistants, technicians, and students to ensure safe, efficient operations. Success in this role requires seamless collaboration with physicians, nurses, and the broader interdisciplinary team to coordinate care plans that align with the patient's physical, emotional, and intellectual capabilities. You will also drive your own professional growth through continuous education and mentorship opportunities.
