About the Role

This position serves as a vital link in the rehabilitation continuum, responsible for the daily coordination and delivery of physical therapy services. You will implement specific treatment programs for individual patients and patient groups, adhering to the principles and practices of physical therapy and established department objectives. The role is designed for a licensed Physical Therapist or Occupational Therapist, with lymphedema experience greatly preferred.

In this role, you will evaluate patients by gathering data from records, interviews, and observations to develop and implement individualized treatment programs. You will provide direct therapy services within various functional settings, planning interventions that align with each patient's physical, emotional, and intellectual capabilities. Collaboration is key; you will communicate effectively with patients, families, physicians, and interdisciplinary team members to ensure a cohesive care plan. Additionally, you will maintain comprehensive documentation and actively pursue professional growth through educational programs and workshops.
