This position serves as a vital extension of our care team, providing essential physical therapy interventions to designated patients under the direct supervision of a Physical Therapist (PT). You will be the face of VNS Health in patients' homes and various care facilities, delivering compassionate care that supports recovery and independence.
In this role, you will follow established plans of care, modifying specific treatment procedures within your scope of practice as a Physical Therapist Assistant (PTA). Your day involves monitoring patient progress through careful observation, documenting all care provided, and maintaining open communication with the supervising PT. You will collaborate with interdisciplinary teams during meetings and training programs to ensure the highest standards of care.
The work environment is dynamic and varied. You will travel to patients' homes, walk-ups, care facilities, and other settings, adapting to different environments and challenges. The role requires physical engagement, including positioning, moving, and transferring patients of varying sizes and conditions, as well as utilizing durable medical equipment. You will also transport necessary technology and supplies, ensuring you are equipped to deliver comprehensive care wherever it is needed.
Candidates selected for this role will be expected to hold a valid New York State Certification as a Physical Therapist Assistant and an Associate's Degree from a New York State approved program. A minimum of two years of clinical patient care experience is required per NY State Education Law. While a valid driver's license and bilingual skills may be required depending on operational needs, basic PC skills are preferred.
