Key Responsibilities
- check_circleDocument all care provided and maintain records per VNS Health policy and standards
- check_circleManage assigned caseload and provide physical therapy services as directed by PT plan
- check_circleUtilize durable medical equipment including wheelchairs, lifts, and exercise equipment
- check_circleTravel to patient homes and facilities to deliver direct care in varying environments
- check_circleFollow established physical therapy plan of care and modify treatment procedures within PTA scope
- check_circleParticipate in interdisciplinary team meetings, in-services, and caseload reviews
- check_circleMonitor patient progress through observation and report findings to supervising therapist
- check_circleAssist patients with therapeutic interventions, transfers, lifting, and activities of daily living
Requirements
- verifiedNew York State Certification as Physical Therapist Assistant
- verifiedAssociate's Degree in Physical Therapist Assistant from a New York State approved program
- verifiedMinimum of two years experience in clinical patient care
- verifiedValid driver's license (may be required)
- verifiedBilingual skills (may be required)
- verifiedBasic PC skills (preferred)
