This position serves as a vital extension of our care team, providing physical therapy interventions to designated patients under the direct supervision of a Physical Therapist (PT). You will play a key role in monitoring patient progress through careful observation and maintaining open communication with your supervising therapist. Working in accordance with New York State Education law, you will deliver compassionate care directly in patients' homes and varying community facilities.
Your day will involve traveling to diverse environments, from elevated buildings and walk-ups to single-family homes and care facilities. You will manage an assigned caseload, delivering physical therapy services and patient education as directed by the established plan of care. This role requires active participation in interdisciplinary team meetings, in-service training, and caseload reviews to ensure the highest standards of care.
