About the Role
We are seeking a dedicated Physical Therapist Assistant to join our Home Care team serving patients in the Bronx. In this role, you will work under the supervision of a licensed Physical Therapist to deliver high-quality, patient-centered rehabilitation services directly in patients' homes. You will implement established treatment plans, provide therapeutic exercises, and assist with functional mobility training to improve safety and independence in daily living.
As a PTA with VNS Health, you will visit patients across diverse Bronx neighborhoods, performing thorough observations and reporting changes in condition to the supervising Physical Therapist and interdisciplinary team. You will educate patients and caregivers on home exercise programs, safe transfers, assistive device use, fall prevention, and energy conservation, tailoring instruction to each patient's goals, cultural background, and home environment. This position is ideal for a compassionate, detail-oriented clinician who values autonomy in the field and the opportunity to build meaningful relationships with patients and families over time.
