About the Role

This Physical Therapist position focuses on providing outpatient rehabilitation services in an in-home setting for the older adult population. Through our "Powerback Rehabilitation To You" model, you will evaluate and treat patients in their most functional environment—whether that is a private residence or a room within an independent living facility.

You will be part of a flexible schedule and work environment, assessing patient needs to develop and deliver individualized therapy programs designed to restore, reinforce, and enhance performance. Your role involves instructing and training patients and caregivers in skills essential for promoting independence, while collaborating with other disciplines to address comprehensive patient needs.
