As a Physical Therapist, you will provide essential rehabilitation services to patients within a home health or community setting. Working under the direction of the Clinical or Rehab Director, your primary mission is to assess, evaluate, and treat patients according to physician-approved plans of care. You will play a pivotal role in helping patients regain functional independence, whether through direct treatment, training families in the use of therapeutic appliances, or coordinating with other home health services.
Your work involves a dynamic mix of clinical assessment and patient education. You will evaluate patients' muscle strength, mobility, and gait to determine their potential for rehabilitation and develop comprehensive plans of care. A significant portion of your time will be spent directing treatment, instructing patients and caregivers on therapeutic goals, and potentially training patients in the use of prosthetic devices.
Beyond direct patient care, you will collaborate closely with the broader care team. This includes participating in case conferences, providing inservice education to nursing personnel, and supervising Physical Therapy Assistants and home health aides. You will also be responsible for documenting clinical progress, preparing summaries based on goal attainment, and leading discharge planning efforts.
Candidates must possess a degree from an accredited baccalaureate or master's program in physical therapy and hold a valid state license. Two years of experience is required, with community or home health experience preferred. Applicants must also maintain current CPR certification and possess a valid driver's license with an insured, reliable vehicle to travel to patient locations.
