PACE (Program of All-Inclusive Care for the Elderly) offers a distinct work environment compared to traditional clinical settings. This role focuses on providing physical therapy services to program participants across Day Centers, Nursing Homes, or in-home settings. The position is designed to foster meaningful relationships with patients, allowing you to get to know them deeply while delivering acute, restorative, or maintenance care.
You will function as a key member of the Interdisciplinary Team, integrating treatment plans into the broader care strategy. The role involves conducting comprehensive assessments, collaborating on 24-hour care delivery, and acting as a liaison with nursing home and assisted living staff. We strive to create thoughtful, caring, and flexible work environments that support a strong work-life balance.
To join our team, candidates must successfully clear a background check and provide medical clearance for communicable diseases, including up-to-date immunizations, prior to having direct participant contact. A valid driver's license and means of transportation are required for this position.
