Key Responsibilities
- check_circleReview, verify, and process medication orders for accuracy, appropriateness, and safety
- check_circleDispense medications in accordance with federal, state, and facility regulations
- check_circleProvide patient counseling on medication use, side effects, interactions, and adherence strategies
- check_circleCollaborate with physicians, nurses, and clinicians to optimize medication therapy and patient outcomes
- check_circleConduct medication therapy management and support transitions of care to prevent medication-related problems
- check_circleMonitor and manage medication inventory, including controlled substances, ensuring proper storage and recordkeeping
- check_circleDocument all pharmacological interventions and activities accurately in electronic health records and pharmacy systems
- check_circleIdentify, evaluate, and resolve drug-related problems, including potential interactions, duplications, and contraindications
- check_circleParticipate in quality and safety initiatives related to medication use, such as antimicrobial stewardship or formulary management
- check_circleAdminister immunizations where permitted and monitor for adverse drug events
Requirements
- verifiedActive and unrestricted Pharmacist license (state-specific)
- verifiedDoctor of Pharmacy (PharmD) from an ACPE-accredited program
- verifiedImmunization certification (e.g., APhA Pharmacy-Based Immunization Delivery) preferred
- verifiedBasic Life Support (BLS) certification preferred
- verifiedBoard certification (e.g., BCPS, BCACP) is a plus
