As a Physical Therapist, you will use discretion and independent judgment to provide age-appropriate services for hospice patients. You will be part of an environment that challenges you, offering clear direction on what it takes to succeed while providing development opportunities for other roles you may be interested in. Your work will focus on symptom control and enabling patients to maintain activities of daily living and basic functional skills through comprehensive assessment, direct treatment, and the development of home exercise programs.
You will serve as a vital link within the interdisciplinary team, participating in group meetings to provide input on patient plans of care. A key part of your day involves instructing patients, families, and the care team on functional limitations and progress. You will also manage documentation according to hospice standards, prepare written discharge summaries, and ensure seamless communication with physicians and administrators regarding patient status and comfort levels.
