As a Physical Therapist in Home Health, you will be the primary driver of patient care within the home setting. Your role involves completing comprehensive assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on these findings, you will collaborate to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and continuously evaluate the patient's progress.
You will work within the scope of practice defined by state laws, coordinating closely with other members of the healthcare team, including supervising nurses and physicians. A key part of your day involves instructing and informing the patient, family, and caregivers regarding the plan of care, functional limitations, and progress toward goals. You will visit patients according to the Plan of Treatment, ensuring all progress notes are completed and submitted in accordance with LHC policy.
