The Patient Care Support (PCS) Companion, also known as a Patient Observer, serves as a dedicated safety partner for patients at risk of unintentional or intentional harm. This role is critical for individuals who may be confused, experiencing withdrawal symptoms, at high risk for falls, or prone to elopement. The Companion provides constant, one-on-one observation to ensure the patient's safety while allowing the clinical team to focus on medical treatment.
You will function under the direction of the VPNE alongside the unit and clinical team, acting as the eyes and ears for the nursing staff. Your primary mission is to maintain a safe environment by monitoring the patient's behavior, preventing escalation, and ensuring the patient remains in sight at all times. Whether accompanying a patient to the bathroom, managing bed alarms, or simply engaging in conversation to de-escalate anxiety, your presence is the first line of defense in patient safety.
Your shift begins with a thorough handoff from the Registered Nurse to understand the specific needs and risks associated with your assigned patient. Throughout your shift, you will adhere to strict patient conversation guidelines and maintain a professional, courteous attitude toward patients, families, and colleagues. You will be responsible for documenting patient behavior accurately and ensuring that personal belongings are secured to prevent self-harm.
Safety is at the core of every action you take. You will demonstrate proficiency with safety equipment, maintain proper body mechanics, and understand your role during hospital codes. You will remain on duty until officially relieved by oncoming coverage or until the nurse discontinures the observation order. Success in this role requires the ability to interact with sensitivity and respect for diverse populations, conveying the values of patient and family-centered care.
This is an entry-level position requiring proficiency in English, both verbally and in writing. Previous service industry experience is preferred. Candidates must demonstrate a strong commitment to safeguarding patient privacy and security, adhering to all hospital policies and procedures. If you identify potential compliance issues, you are expected to report them immediately to a manager, supervisor, or director.
