COMCARE, a department within Sedgwick County, is dedicated to providing comprehensive, goal-directed, and patient-centered services. The team supports children with Serious Emotional Disturbance and their families, aiming to improve their quality of life and ability to function successfully within the community.
This position serves as a vital link in the continuum of care, delivering Psychosocial Rehabilitation and Attendant Care. You will facilitate self-contained, goal-directed individual and group treatments designed to help patients minimize the effects of mental and emotional impairments. Through these activities, you will assist individuals in daily problem-solving, social skill development, leisure training, and enhancing personal relationships.
A significant portion of the role involves providing one-to-one support and supervision to maintain patients in natural community settings, such as their homes, schools, or local community spaces. You will offer direct support for activities of daily living and help families maintain the daily routines critical to a stable lifestyle. Additionally, you will utilize your personal passenger vehicle to transport patients to medical appointments and therapeutic activities, including access to community resources, employment, education, and social activities.
Beyond direct care, you will play an active role in treatment planning and care coordination. This includes collaborating with patients, families, and legal guardians to develop treatment plans, as well as advocating for and arranging services with other agencies. You will work to expand relationships with external partners through memorandums of understanding and by utilizing community resources to their fullest extent.
Applicants must meet specific licensing standards and pass required background checks, including KBI, DCF child abuse checks, adult abuse registry, and motor vehicle screens. Selected candidates will complete orientations provided by Sedgwick County and COMCARE before beginning their duties.
