About the Role
As a Case Manager RN, you will be the vital link supporting patients and families through every step of their care journey. This role exists to ensure safe transitions, clear communication, and consistent quality across the continuum of care. You will partner with physicians, nurses, and department leaders to coordinate services and connect resources, developing care plans that reflect each patient's unique needs.
Your day will involve guiding patients through program orientations, explaining rehabilitation philosophies and insurance benefits, and facilitating family conferences. You will collaborate closely with the Rehab Program Director and interdisciplinary teams to implement individualized treatment plans that honor patient strengths and recovery goals. Through every interaction, you will promote HCA Healthcare's core values of compassion, respect, and excellence.
