About the Role

As a Case Manager RN, you will be the guiding force for patients and families throughout their entire care journey. This role exists to ensure safe transitions, clear communication, and consistent quality across the continuum of care. You will partner with physicians, nurses, and department leaders to coordinate services, connect resources, and develop care plans that reflect each patient's unique needs.
