About the Role
This position is dedicated to providing evidence-based mental health services that promote continuity of care for individuals within their own residences and the broader community. As a vital member of Communicare's treatment team, you will serve as a bridge between clinical care and the family unit, working directly with clients and their families to ensure active participation in the treatment plan.
Your day will involve managing an active caseload while coordinating with referral sources, outside agencies, and internal programs to maintain a seamless system of care. You will be responsible for rapid response crisis services, intakes, and therapy, all while ensuring strict adherence to Department of Mental Health and Division of Medicaid standards. Success in this role requires the ability to work independently, navigate the community effectively, and collaborate positively with diverse stakeholders, including families, staff, and community members.

Requirements
