This position is dedicated to providing evidence-based mental health services that promote continuity of care for individuals within their own residences and the broader community. The role serves as a vital link in a coordinated system of care, requiring the incumbent to work closely with families, referral sources, and other programs to ensure comprehensive treatment plans are effectively implemented.
As a member of the treatment team, you will manage an active caseload while arranging for medication evaluation, management, and monitoring. The position involves conducting rapid response crisis services, intakes, and therapy sessions as directed. A key aspect of the role is assessing home and family situations to ensure active family participation in the treatment process, while maintaining strict adherence to Communicare, Department of Mental Health, and Division of Medicaid standards.
Candidates must successfully complete a background check and drug screen, as Communicare maintains a drug-free workplace. Selected applicants will be required to complete specific training in crisis intervention, including CPI, suicide prevention, pre-evaluation screenings for civil commitments, and RELIAS, alongside other trainings deemed necessary by the organization and state departments.
