About the Role
The Outpatient RN CAP 1 position serves as an entry-level designation within the Clinical Advancement Program (CAP). As a vital member of the End Stage Kidney Disease (ESKD) healthcare team, you will provide and coordinate clinically competent care, encompassing assessment, planning, intervention, and evaluation for an assigned group of patients. This role is designed to foster growth through participation in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
In this capacity, you will collaborate with the interdisciplinary team to ensure the safe and effective delivery of hemodialysis therapy. Your day-to-day involves systematic data collection and analysis before, during, and after treatments, allowing you to make necessary adjustments to treatment plans. You will also play a key role in patient education, assessing learning needs to provide ongoing guidance to patients and their families regarding transplant options and treatment modalities.
