As an Outpatient RN CAP 1, you are stepping into an entry-level designation within the Clinical Advancement Program (CAP). This role is designed for Registered Nurses with less than two years of nephrology experience who are eager to grow within a supportive environment. You will serve as a vital member of the End Stage Kidney Disease (ESKD) healthcare team, accountable for the provision and coordination of clinically competent care for an assigned group of patients.
Your day-to-day involves the systematic collection and analysis of patient data before, during, and after hemodialysis treatments. You will collaborate with the interdisciplinary team to assess patient needs, adjust treatment plans, and provide essential education to patients and their families regarding transplant options and treatment modalities. Beyond direct patient care, you will participate in decision-making, quality improvement activities, and staff training, helping to enhance both patient outcomes and facility operations.
