As a Home Health Occupational Therapist, you will become a vital part of our caring community. Reporting to the Home Health Branch Administration, your primary focus is to help patients regain strength, mobility, and independence from the comfort of their own homes. You will deliver personalized care that emphasizes rehabilitation and functional improvement, empowering individuals to overcome physical limitations and perform everyday activities with confidence.
In this role, you will assess patients' daily living and work-related skills, developing therapeutic retraining programs with measurable objectives. You will administer diagnostic tests to assist physicians in evaluating functional levels and collaborate with the patient's physician and the broader healthcare team to develop and revise Plans of Care. Your work involves providing therapeutic treatment to restore strength and function, consulting with vocational team members to create vocationally oriented plans, and designing community reintegration activities to support both physical reconditioning and psychological adjustment.
You will also play a key role in care coordination and discharge planning, serving as a resource to other healthcare personnel. This includes recommending and designing special adaptive equipment to improve clients' environments and training staff and family members on retraining programs for daily skills like dressing, feeding, and grooming. Additionally, you will supervise and evaluate the performance of Occupational Therapy Assistants and Home Health Aides while ensuring accurate and thorough documentation of all patient care observations and interventions.
This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. As this position involves travel to patient homes, it is also part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
