In this role, you will partner with the patient's physician and other health care team members to develop and revise plans of care. You will provide therapeutic treatment to improve strength, coordination, and function, while also consulting with vocational team members to develop plans consistent with patient needs. Your day will involve designing community reintegration activities, recommending adaptive equipment, and training staff and family members to carry out retraining programs. You will also supervise Occupational Therapy Assistants and Home Health Aides, ensuring accurate documentation of care observations and interventions.
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
As this position requires travel to patient homes, it is part of Humana's driver safety program. Selected candidates must maintain a valid state driver's license and personal vehicle liability insurance in accordance with their residing state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
