This position is specifically for the lower Cape (Brewster, Orleans, Chatham, Eastham area).

About the Role

As a Home Health Occupational Therapist, you will partner with patients and the healthcare team to assess daily living and work-related skills, developing therapeutic retraining programs with measurable objectives. Your day will involve administering diagnostic tests, conferring with physicians to revise Plans of Care, and providing therapeutic treatment to restore strength, coordination, and function.

You will act as a resource for care coordination and discharge planning, designing community reintegration activities to assist clients in physical reconditioning and psychological adjustment. Additionally, you will supervise and instruct Occupational Therapy Assistants and Home Health Aides, ensuring high-quality service delivery while documenting care observations and interventions thoroughly.

Additional Information

This role is patient-facing and part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. As this position involves driving, it is part of Humana's driver safety program. Candidates must maintain a valid state driver's license and personal vehicle liability insurance in accordance with state minimum required limits or specific coverage thresholds.
